Our class will be using computers on a regular basis to complete projects and other assignments.  These assignments often comprise a large portion of the student’s grade. 

• This form was made available to students in my class during their first week of school at PPHS. 
• Student must sign, obtain parent signature and return the form to me prior to using the Internet. 
• This form is good for all 4 years at PPHS.  If you already submitted a completed form while at PPHS, you don’t need to submit a new one.)
